Thesis/Dissertation Signature Request Form

If physical signatures cannot be obtained, signature pages for theses and dissertations can now be created and signed using GC Signatures, an online signature service. The link below has been set up for students to create a signature page PDF that can be submitted to everyone who needs to sign it.
 

Before you begin

  • Check with Department: Contact the chair of your committee and the chair of your department about any procedures or deadlines that have been set up about what is expected from you to receive the final signature and to verify the use of the electronic signature form
  • Gather Signature Info: The signatures required for the signature form vary by degree. Based on the college and department selected on the first page of the form, the second page will indicate the roles/signatures required for your submission.
  • Inform the Committee: Consider contacting the Committee before submitting the Signature Request Form to expect an email from GC Signatures with the subject: Thesis/Dissertation Signature Request for [Your Name]
