- Event organizers should type or print the information in the form first (i.e. program/event Name and Date)
- Print out as many as needed.
- Do not change, delete, or add questions/responses.
- All responses should be stapled and sent, or scanned to, Lamonica Sanford at lamonica.sanford@gcsu.edu.
